The right jowl treatment depends on your stage. Early softening along the jawline may respond to Sofwave skin tightening, lost structure along the jaw may suit filler, and tissue that has begun to descend may suit a thread lift. Once jowls are heavy and well established, surgery is usually the more honest answer.
We assess which stage you are at before suggesting anything, and we will tell you plainly if a non-surgical option is unlikely to meet your expectations.

Several gradual changes that add up along the jawline.
Jowls are the softening of tissue along the lower edge of the face, just beside the chin, that blurs a once clean jawline. They rarely have a single cause. More often, several gradual changes add up:
Which of these is driving your jowls decides what is likely to help. A treatment that firms skin will not replace lost volume, and filler cannot tighten skin that has loosened. That is why we start with an assessment rather than a treatment name, and why every plan here is made under the medical direction of Dr. Andrea Garcia, MD CCFP.

A ladder of options, not a single answer.
Non-surgical treatment can soften jowls and improve the line of the jaw. It does not erase them, and the realistic aim is a meaningful, natural improvement. The options, roughly from earliest stage to latest:
Many people sit between two rungs, and a plan may combine options. Because filler shows a change the same day while Sofwave works gradually over weeks to months, the order matters, and your provider will explain which fits you and in what order.

For early laxity, with realistic expectations.
Where jowling is early and the main issue is skin that has softened, Sofwave is a sensible first conversation. It delivers ultrasound energy about 1.5 mm into the skin while the surface is cooled, and the new collagen that follows may firm the lower face gradually over weeks to months.
The evidence for the lower face is encouraging but modest. A study of 36 people treated once reported a statistically significant reduction in the depth of the nasolabial folds and marionette lines at one and three months. Another study found that a trend toward reduced volume under the jaw was not statistically significant, so Sofwave is not a way to slim the jawline. Participants in the lower face study rated discomfort at an average of about 6.6 out of 10, so comfort measures are discussed with you beforehand.
According to the manufacturer, a Sofwave appointment generally takes about 30 to 45 minutes. Many people have a single treatment, and your provider will advise whether more may help. The main published trial included a wide range of skin tones, and the reactions most often reported afterwards were temporary redness and occasional mild swelling. Sofwave does not add volume and does not reposition tissue that has dropped. For the full timeline, see what Sofwave results look like over time, or visit our main Sofwave page. Choosing between the non-surgical options.
These treatments are often compared as rivals, but they do different jobs. Filler restores structure where volume has reduced. Threads reposition tissue modestly. Sofwave firms skin gradually. The right choice depends on which change is most responsible for your jowls, and sometimes more than one plays a part.
Sometimes the jawline is best supported from the chin, and chin fillers may be discussed where that applies. Where general firmness and volume have faded across the lower face, Sculptra works gradually by stimulating collagen. If the neck is part of your concern, our guide to neck tightening explains the options there.
